SAŽETAK
Britanski anesteziolog dr. Archie Brain je izumio laringealnu masku 1983. godine. Laringealna maska (LMA) služila je kao alternativno, supraglotično sredstvo za obezbjeđivanje disajnog puta i u upotrebi je od 1988. godine. Laringealnu masku koristi anesteziološki tim za: ventilaciju pacijenata tokom anestezije, u slučajevima nedovoljne ventilacije, pri oživljavanju kada se ne može izvršiti ventilacija pacijenta drugim načinom, kratkotrajnu anesteziju pacijenata koji dišu spontano. Postavlja se “slijepom” metodom, niz tvrdo nepce prateći njegovu fiziološku krivinu, obavezno zadnju stranu namazati nekim lubrikantom (npr. lidocain gel), namazati cuff sa lubrikantom da ne bi jezik otežavao postavljanje LMA, cuff se u potpunosti isprazni i oblikuje. Postavlja se u hipofarinks, napuhavanjem cuff-a zatvara se prostor oko glotisa, koji se nalazi se na raskrsnici respiratornog i digestivnog trakta. Idealna pozicija LMA je:baza jezika na gornjem rubu, gornji ezofagealni sfinkter na donjem rubu. Pri najboljoj poziciji maske moramo znati da ona ne štiti disajni put od aktivne aspiracije, a štiti od neaktivne aspiracije. auskultacija pluća kapnografija provjerava se strujanjem zraka vanjskim slušanjem oko usta, provjeriti eventualni porast volumena želudca. Kontraindikacije su: pacijenti sa velikim rizikom od aspiracije (pun stomak, ileus, hematemeza, dijabetičari, urgentne operacije..), pacijenti sa glotičnom ili subglotičnom opstrukcijom, veoma ograničeno otvaranje usta ili ekstenzija vrata, trudnice od 14-16 nedelje, pacijenti sa višestrukim i masivnim traumama, maksilofacijalne traume, potrbušni položaj, operacije duže od 2 sata. LMA je najefikasnije alternativno supraglotično sredstvo, lako za održavanje prohodnosti disajnih puteva, pogodna za sve vrste pacijenata. Ideja anesteziologa dr.Archie Brian-a je do sada korištena na preko 300 miliona pacijenata širom svijeta i bez sumnje je spasila mnoge živote.

APPLICATION OF THE LARYNGEAL MASK IN ANESTHESIA

SUMMARY
British anesthetist Dr. Archie Brain invented laryngeal mask in 1983. Laryngeal Mask (LMA) was used as an alternative, supraglottic means for securing the airway in use since 1988. Laryngeal Mask is used by anesthetic team for: ventilation of patients during anesthesia in cases of insufficient ventilation, to revive patient when its not possible to execute ventilation of the patient in any other form, short-term anesthesia of patients who breathe spontaneously. The “blind” method sets a series of hard palate following its physiological curve, insuring to enlarge rub with some lubricant (eg. Lidocaine gel), smear cuff with a lubricant that would not have made the tongue setting of the LMA, cuff is completely empty and shapes. Placed in the hypopharynx, inflating cuff-and closes area around the glottis, which is located at the crossroads of the respiratory and digestive tract. The ideal position of the LMA is the base tongue at the top edge, upper esophageal sphincter at the lower edge. The best position masks must be known due it doesn’t protect the airway from aspiration of active and inactive aspiration. Auscultation of capnography is checked the air flow around the outside listening to mouth, also check the possible increase in the volume of the stomach.
Contraindications are: patients with a high risk of aspiration (full stomach, ileus, haematemesis, diabetes, emergency operations), patients with Glottic or subglottic obstruction, with very limited opening of the mouth or neck extension, pregnant women from 14-16 weeks, patients with multiple and massive trauma, maxillofacial trauma, prone position, operations that are longer than 2 hours. LMA is the most efficient alternative supraglottic device, it is also easy to maintain airway patency and suitable for all types of patients. The idea of Anesthesiologists dr. Archie Brian’s has so far been used by over 300 million patients worldwide and undoubtedly saved many lives.
